Passports
A passport is an official document issued by the federal government that verifies a person’s identity. Washington County has four locations designated by the U.S. Department of State as official U.S. Passport Acceptance Facilities.
Online resources and forms are available at travel.state.gov.
Schedule an Appointment
Appointments are strongly recommended for both passport applications and photos. Walk-in service is based on availability.
- Appointments required for Cottage Grove Service Center (all passport services).
- Please arrive on time so we can honor your appointment.
Apply in person (form DS-11) if:
- You are applying for your first passport, or;
- Your passport was lost, stolen, or damaged, or;
- Your passport was issued more than 15 years ago, or;
- You are renewing a 5-year passport, or;
- You are applying for a child under 15
What to bring:
- Completed Form DS-11 (fill out online or at our office; do not sign before your appointment. Use black ink only. No white-out.)
- Proof of US Citizenship (original or certified copy and a photocopy). NOTE: Documents must be submitted at time of application. Your original documents will be returned after receiving your passport.
- Photo Identification (original and photocopy).
- Passport Photo meeting U.S. Department of State standards. Washington County can take your photo for $18 per person (includes two photos).
- All applicants present, including minors
Payment:
- Check or money order payable to U.S. Department of State for passport fees.
- Processing fee ($35) payable to Washington County by cash, check, or credit/debit card (2.15% service fee).
- Passport Photos: ($18) cash, check, or credit/debit card (2.15% processing service fee)

Please be sure to bring the following items when you apply for a passport. If you do not have all the required items, your application cannot be processed, and your appointment will be rescheduled. All applicants (including minors) must be present at the appointment. Both parents and/or legal guardians of children under age 16 must appear in person. Visit travel.state.gov for allowable exceptions.

- Use black ink only.
- Do not use white-out.
- Do NOT sign the form.
- Printable applications available online at Form DS-11 (PDF) (printed single-sided).
- Application forms are also available at our office, but please arrive early for your appointment to ensure enough time to complete the form.
Proof of U.S. Citizenship
- Must show an original or certified copy.
- Must provide a black and white photocopy of the document.

Passport Photo
- Must meet requirements set by U.S. Department of State.

Visit any Washington County Service Center to have your passport photos taken.
- Cost: $18 per person for 2 color passport photos.
- Payment Options: Cash, check or money order payable to "Washington County," or credit/debit card (for an additional 2.15% fee).

Renew your passport online or by mail. However, if you are coming in for photo services, please schedule an appointment. Visit How to Renew or Replace a Passport for information.
Renew Online: Eligible U.S. citizens who are applying for routine service can now renew their passports online.
Renew by mail (form DS-82) if your passport:
- Is undamaged (other than normal wear) and can be submitted with your application
- Has not been stolen or lost
- Was issued 15 years ago or less and valid for 10 years
- Was issued when you were 16 or older
- Was issued in your current name (or you can show proof of name change, such as a marriage certificate)
