Disability Parking Certificates and License Plates

Apply for disability parking certificates and license plates at any Washington County License Center location. To qualify for a disability parking certificate, you must submit an application completed by your physician. 

A temporary 90-day certificate will be issued upon returning your completed application. Disability plates and certificates are mailed to you by the State of Minnesota. If you do not receive your placard or plates within 90 days, please contact the Minnesota Department of Public Safety at 651-297-3377.

Apply for a Disability Parking Certificate

Disability parking certificates are issued to individuals (drivers or non-drivers) and to institutions that transport persons who have disabilities. Certificates are transferable to any vehicle transporting a person with a disability.

Apply online, print a Disability Parking Application (PDF) or obtain an application from any Washington County License Center. Your medical provider must complete and sign the health professional medical statement section of the application.

Renew a Disability Parking Certificate

If you are renewing an expiring permanent (six-year validity) certificate, submit the renewal card sent by the State of Minnesota or complete an application (PDF). The applicant's signature is required, the physician's signature is not required. For all other expiring certificates, (temporary, short-term or long-term), signatures are required from the physician and applicant. 


Fees
