Apply for a license

1. Print and complete the forms:
   • Precious Metal Dealer License Application (Private Data) (PDF).
   • Precious Metal Dealer License Application (For Public Use) (PDF).
   • Proof of Workers' Compensation Insurance Coverage (PDF).
   • Affidavit of Sole Proprietorship (PDF).
2. Notarize the application.
3. Obtain proof of Workers' Compensation Insurance Coverage or Affidavit of Sole Proprietorship.
4. Mail the completed and notarized forms, and proof of insurance to Washington County.

Cost

The license fee is $25, payable by cash or check. Please do not send cash through the mail.
